Welcome to AdCare Living
We are pleased to welcome you to AdCare Living, an assisted living facility dedicated to providing our residents with a comfortable and supportive environment. These Terms and Conditions (the "Terms") outline the rules and regulations that govern your residency at AdCare Living.
Please read these Terms carefully before signing.
1. Admission
- To be eligible for admission, you must be at least 55 years of age and require assistance with some activities of daily living (ADLs), such as bathing, dressing, or eating.
- You must also be able to ambulate independently or with minimal assistance.
- A pre-admission assessment will be conducted to determine your eligibility and care needs.
2. Residency Agreement
- Upon admission, you will be required to sign a Residency Agreement. The Residency Agreement will specify your monthly rent, the services you will receive, and the terms of your residency.
- Please read the Residency Agreement carefully before signing.
3. Monthly Rent
- Your monthly rent will cover your accommodation, meals, and basic care services.
- Additional fees may apply for certain services, such as medication administration or transportation.
4. Services
AdCare Living offers a variety of assisted living services, including:
- Assistance with ADLs
- Meals
- Housekeeping
- Laundry
- Medication administration
- Transportation
- Social and recreational activities
5. Medication Administration
- AdCare Living will administer your medications according to your doctor's orders.
- You must provide AdCare Living with a list of all your medications and their dosages.
- You must also inform AdCare Living of any changes to your medication regimen.
6. Transportation
- AdCare Living may provide transportation for medical appointments, errands, and social outings.
- Transportation must be scheduled in advance.
7. Activities
AdCare Living offers a variety of social and recreational activities for residents.
Activities may include:
- Exercise classes
- Games
- Crafts
- Field trips
- Movie nights
8. Pets
- Pets are not permitted at AdCare Living with the exception of service animals.
- Service animals must be registered with the assistance of a trained professional.
9. Visitors
- Visitors are welcome at AdCare Living during visiting hours.
- Please inform the staff if you are expecting visitors.
10. Discharge
You may be discharged from AdCare Living if:
- You no longer require assisted living services.
- You become a risk to yourself or others.
- You violate the Terms and Conditions of your residency.
11. Grievance Procedure
- If you have any concerns or complaints about your stay at AdCare Living, please follow our grievance procedure.
- You may speak to a staff member, write a letter to the Administrator.
12. Emergency Procedures
- In case of an emergency, please follow the instructions of the staff.
- Emergency evacuation procedures are posted throughout the facility.
13. Changes to Terms and Conditions
- AdCare Living reserves the right to make changes to these Terms and Conditions at any time.
- You will be notified of any changes in writing.
